The Application Checklist is provided to assist families in gathering all college application materials that must be given to guidance at least two weeks prior to the application deadline.
The Responsibilities checklist lists the many pieces of the college application process indicating responsibilities of the student, the parent, and the guidance.
The College Rep Permission form must be filled out before each student meets with college representatives visiting at the high school.
The Fall College Profile form should be filled out by students to inform guidance of their college application plans.
Applying
The College Materials Checklist must accompany the college application materials given to guidance at least two weeks prior to the application deadline.
The Senior Data Sheet must be completed by the student and given to guidance to be used to complete the guidance package to be sent to the colleges.
The Transcript Request form must be filled out and submitted to guidance at least two weeks prior to the college application deadline.
The Permission to Release Information form must be filled out prior to guidance releasing official transcripts, temporary records, or special education records to any institutions.
The Parent Information form is available for parents to provide information to help the guidance department present the best student profile possible for your child.
The Background Information form is available for students to provide information about themselves to help the guidance counselor write a good letter of recommendation.
